Choose a real failure where your decision contributed and there were consequences.

## Your Task

1. Write it with the five steps from section 2, labelling each step.
2. Check the ratio: is the failure itself at most a third of the words?
3. Name the specific system change (test, alert, checklist, process) you made.
4. Add one later situation where the lesson helped you.
